Notes:

1) The three controls with unknown tobacco amount or age under 25
   were deleted for the Chapter 6 analyses.

2) The grouped data in Appendix I do not agree exactly with these
   continuous data as regards the alcohol and tobacco groups, due
   to a programming error made in the 1970's.
