Welcome to the Chronic Care Clerkship - Boise Experience

**Please complete the following UW Medical Student Clerkship Form and submit to Lynn Kelley at Boise Physical Medicine & Rehabilitation at least TWO WEEKS before the start of the rotation.

Please contact Lynn Kelley (boisephysicalmed@qwest.net) at BPM&R at least one week prior to your clerkship starting. Lynn will provide you with a schedule.
