Chronic Care Clerkship - Conjoint 690

Student Requirements

Didactic Sessions

You will attend both didactic sessions at the University and be actively involved in discussions and other learning activities during the day. Sessions will include didactics and discussions involving all students in the rotation.

Clinical Experiences

You will be involved in clinical activities within your focus area. This may include time spent in wards, in outpatient clinics, working with a hospice team in the community, completing home or facility visits, involved in web-based learning, and being on-call. All assigned clinics and other clinical experiences are required. Additional clinical experiences could include sessions with a variety of other professionals seeing your patients, including: clinical psychologists, speech and language pathologists, social service workers, physical and occupational therapists, and vocational and educational counselors.

At a minimum, you must participate in chart rounds, consult with a case manager or other social service worker, and conduct a home visit. All required activities, Monday sessions and including call, will take place with in a 50-hour week.

Home Visits

You will be involved in one or more visits to a patient's home or other care facility, such as a nursing home or hospice facility. The patient(s) will be chosen in consultation with your preceptor. Your preceptor or a visiting nurse may accompany you on the visit(s). A brief written report of the visit will be completed and submitted to your preceptor for each visit.

If you have no patient discharges that lend themselves to a home visit, you have two alternate options:

  1. You can assemble the family of a patient who is on your service and interview them at the hospital; you should be able to answer the 14 questions about the interface between the patient and his/her multiple environments, i.e. social, physical, environmental etc. by doing the detailed interview. You must indicate that you obtained this information from the family without visiting their home.
  2. You can negotiate through your attending to do a home visit with one of the visiting nurses. You will be interviewing a patient whom you have not previously met, however other students have found this option was surprisingly rewarding.

Comprehensive Case Write-up

You will complete a major comprehensive write-up of a patient and family using a structured report form. The write-up will be presented to fellow students and faculty on the last day of the clerkship.

Readings and Other Educational Assignments

You will be expected to complete assigned readings and/or web based educational assignments as directed by clinical faculty. A list and schedule of reading assignments can be found in the Resources section of this web site. Readings and other resources for clinical focus assignments are found in their respective web pages.

Presentation of your Comprehensive Case Write-up

You will conduct a 10 to 15 minute presentation followed by a 15 minute discussion of your comprehensive case write-up on the last day of the clerkship. Presentations will be made to your student groups on the last didactic day of the clerkship.
