Elective Description

Welcome to Med 681. This elective was established in 1999 to increase your exposure to Dermatologic Surgery. It is intended for the student with extra interest in dermatology, although is expected to be useful for students expecting to do cutaneous surgery (Family Practice, Plastic Surgery, Otolaryngology).

The elective is intended to provide instruction in and exposure to outpatient ambulatory cutaneous surgery (see objectives below). Participation in a small number of general dermatology clinics with an emphasis on the diagnosis of pigmented lesions may also occur. In addition there will be participation in regularly scheduled dermatology didactic teaching rounds.

Objectives

1. Understand the diagnosis and treatment of the three major skin cancer types: BCC, SCC and Melanoma

2. Develop an approach to lesions on the skin

3. Understand and be able to demonstrate different kinds of skin biopsies

4. Understand the cutaneous anatomy necessary for performing skin surgery

5. Discuss and use safely local anesthetics for skin surgery

6. Understand the indications for and technique of Mohs Micrographic Surgery

7. Understand Practical Wound Healing Issues for Cutaneous Surgery

8. Understand concepts in performing excisions on the skin

  • Principles of Relaxed Skin Tension Lines
  • Choice of Suture materials
  • Principles of Excisional Surgery
  • Intra and Postoperative Management

9. Discuss the surgical options for dealing with acute surgical cutaneous defects

  • Second Intention Healing
  • Primary Closure
  • Skin Grafts
  • Local Skin Flaps

10. Understand the role of lasers in dermatology

11. Understand basic concepts in dermatopathology

** Please check with dermatology resident assigned to dermatologic surgery or with chief resident for location and times of didactic teaching. Some sessions will be of interest to you and some may not be. Please check weekly with the resident.

    • Excellent and comprehensive online lectures are found on this website and archived dermatologic surgery lectures with audio are available at www.mdlive.net
    • Please also review the online annotated powerpoint lectures on this website
  • Textbook
    • Consider reading select chapters of the book: "Ratz JL et al. Textbook of Dermatologic Surgery. Lippincott-Raven, 1998". This book is available in the clinic and may be borrowed overnight if not in use by the dermsurgery resident. Alternatively, select chapters may be photocopied for reading. The book is also available in the library.

Optional Activities

1. You will be invited to participate in any concurrent dermatologic teaching events occurring during your rotation.

2. You are welcome to bring in pig's feet during any of the days assigned to Dr Berg. Instruments and a Workshop Manual will be provided to allow you to practice your suture techniques.

Requirements for Honors

Evidence of special interest (in addition* to outstanding performance and attendance in the rotation) will be required to obtain an overall honors grade. This may take a number of forms, including:

  • Participation in a scholarly project such as preparation of a case report, or assistance in an ongoing project.
  • Evidence or Literature-Based Review of a topic of interest.
    • This should be picked in the first 1-2 weeks of the rotation and may revolve around an interesting case or a common problem or question. Other topics may be considered on discussion with Dr Berg.
    • The topic will be presented at the end of the rotation as an informal 15 to 30 minute presentation (with handout) to Dr Berg and the Dermatologic Surgery Resident.
      • More in-depth or longer presentations may be also scheduled subsequently as part of regular Wednesday morning dermatologic surgery didactic teaching for the residents (these are 45 - 50 minute talks with Power Point Slides).

    * NOTE: Although evidence of special interest is required to obtain honors, please be aware that it does not guarantee an honors grade.
