Applying for the 2009-2010 Fellowship

All eligible candidates are invited to submit an application for the program. Candidates are eligible to apply if they meet the following criteria:

1. Will have successfully completed a ACGME-accredited dermatology residency program by July 1, 2009; and

2. Are a graduate of a U.S. or Canadian Medical School, or are currently ECFMG-certified with either a J-1 visa or US citizenship or permanent residency.
Applications may be downloaded from this website beginning in the spring of 2008. Applications are accepted through the end of August 2008. Interviews are typically scheduled to occur from mid-September to mid-November.

A complete application includes:

1. A one page personal statement explaining your background, interests and current career goals.

2. A formal application form (click here).

3. A current Curriculum Vita.

4. Three or four references, including one from the director of dermatologic surgery at your residency institution, and one from the residency training program director.

5. In addition to your provided references, Dr. Berg may wish to contact other colleagues who have worked with you professionally. In the application form there is a statement which gives general permission for him to contact at his discretion any attendings/colleagues with whom you have worked. If you prefer that Dr. Berg not have this general permission and check "no" on the application form, we will obtain in advance specific permission from you prior to contacting anyone other than your references.

Our Program:

Our program is currently accredited by the ACMMSCO for a one year fellowship. The program is also currently approved by the ACGME as a Procedural Dermatology Fellowship.

Our inaugural fellowship began July 2004. We opted not to take a fellow for 2008-2009.

 

Location:

We are a full-time academic Dermatologic Surgery Clinic in the Division of Dermatology at the University of Washington. The fellowship is under the supervision of Dr. Daniel Berg.

Facilities:

At the UW Dermatology Center outpatient facility, there is a separate Dermatologic Surgery Unit. This consists of 6 Large procedure rooms, 4 examination rooms, a separate surgery patient waiting room and a fully equipped CLIA-certified frozen section laboratory including 2 cryostats and automatic tissue stainer. 2 Rooms are cross-used for both surgery and lasers. Current lasers include Candela GentleLase, GentleYAG and Vbeam as well as a Q-switched YAG and Intense Pulsed Light Device.

Offices in the Dermatology Center are equipped with computers and Ethernet ports and printers so the fellow can retrieve information and have access to medical databases around the clock. The fellow will have a cubicle with desk and computer with fast Ethernet connection. The fellow will have access to the online journal collection of the University of Washington both at work and via home computer connection. Basic textbooks on dermatologic surgery are on-site at the dermatology center. The fellow will also have access to the major medical library located in the Health Science Building on the University of Washington campus. It is a 15 minute walk or 10 minute shuttle ride.

Program Curriculum

The program curriculum encompasses the Didactic Program in combination with the Clinical Experience. Both of these components are described below including a clinic schedule.

Didactic Program

The didactics program consists of three elements:

  • A formal monthly lecture series with scheduled topics presented Wednesday mornings 7:00am to 7:45am. (Click here for schedule)
    • These will be presented by faculty, fellows, residents and occasional medical students. The fellow will play a key role in coordinating these sessions and presenting many of them.
  • The fellow will be expected to review the comprehensive online dermatologic surgery core curriculum lecture series sponsored by MDLive. Lectures with audio are available at their website where they are archived online.
  • During the year, the fellow will be expected to review a comprehensive textbook on dermatologic surgery. The recommended textbook is: Surgery of the Skin. Robinson JK, Hanke CW, Sengelmann R, Siegel D (eds), Mosby 2005.
  • Attendance at the UW Facial Plastic Surgery Anatomy Course (if held)

Clinical Experience:

The majority of the clinical work is cutaneous oncology with an emphasis on Mohs surgery and reconstruction. Approximately 650 Mohs surgery cases are done annually at our institution. Other procedures done include hair transplantation, lasers (Pulsed Dye, Long-Pulsed YAG and Alexandrite, Q-switched YAG and IPL), Injectables (Botox) and less frequently other dermatologic surgical techniques including scar revision and rhinophyma repair. We have an excellent working relationship with other specialties and the fellow may attend selected post-Mohs reconstructions by specialists in Plastics, Otolaryngology and Ophthalmology at the University of Washington affiliated facilities. If possible, some time will be provided to the fellow to work with attendings in these other specialties. As-needed attendance at a Multidisciplinary Tumor Board to present difficult cases is available. Dr Phil Fleckman has a regular nail surgery clinic which you will be able to attend.

The fellow will be intimately and regularly involved in preoperative assessment of patients for surgery.

The fellow will take second call (backing up the dermatologic surgery resident) for surgeries done in the unit when on call. In addition, they will see most of the postoperative patients along with the attending.

Fellows will be initially heavily instructed and supervised as they are given increasing responsibility during the fellowship. As they demonstrate competence in various procedural aspects the supervision will become less-hands on and finally periodic observation will allow the fellow to perform close to independently.

Fellows will review all of the slides for all cases they participate in. In addition the fellow will receive instruction in laboratory management from the chief histotechnologist in the laboratory. Relevant laboratory manuals will be reviewed by the fellow and made available to the fellow as reference.

The fellow will be invited to participate in the annual "Anatomy for Facial Plastic Surgery" Cadaver Dissection course at the University of Washington. Further information about the course (e.g. the 2004 schedule) is available online. This course has been presented by Otolaryngology each year for the past 11 years, with the exception of 2008 and we hope to reinstate this as an annual course (to be announced).

The fellow will be exposed to and assist in cosmetic procedures with Dr. Berg. Currently these procedures include hair transplantation, a variety of lasers, and injectables (fillers and botulinum toxin). In addition some opportunity to visit a nearby private practice for other exposure will be available.

Academic Expectations

The fellow will play an important role in resident teaching both at the bedside and by active participation in monthly dermatologic surgery didactic sessions for the residents. The fellow will be responsible for organizing the annual lecture series in consultation with faculty and the Chief Resident in Dermatology. Further information about the didactic lecture schedule is available at: http://courses.washington.edu/dermsurg/schedule.htm . It will be expected that the fellow will undertake a scholarly project during their year of training. This is intended to be suitable for presentation at a national meeting or publication or both.

Clinical Duties:

The fellow may be asked to be the attending at a weekly dermatology clinic with an emphasis on excisional surgery. The billings from this clinic will help provide the fellow's salary support. This will not be an option if the fellowship is under the auspices of the ACGME.
