When completing road locations/reconnaissance a RECON REPORT is usually completed. In its most basic form it contains the "raw field notes" (see sample field notes). Usually the field notes are updated in the post-field phase with additional comments/observation providing more detailed information than is contained in the raw field notes. Nowadays an additional aspect is that such a report creates an electronic document and is therefore much more easily retrievable than hardcopy field notes, stashed away in some drawer or shoe box.. For examples of recon reports, look at the Washougal Capstone report (1998), Hoodsport (1999) and Toutle (1997).

Adopt a comparable format for your report. Scan the field note pages and include them to the word document as an appendix.
