Case Study 3A 45 year old man presents to your office with complaints of decreased sleep, irritability, difficulty concentrating, excited mood, and increased libido for one week. He is on probation at work after telling the head of his corporate office that she didn't know how to run the business and that he could double their profits in just six weeks. His wife has moved out, saying she can't deal with him any more, but he says that she's just jealous of his brilliance. Four years ago he had a significant depression, with increased sleep, low self esteem, trouble concentrating, anxiety, and hopelessness. He received no specific treatment at that time. He has felt this way in the past once or twice before, but it was milder. He takes a thiazide diuretic for hypertension, but denies other health problems or substance abuse issues. 1.
