Key:
VA = visual acuity
OD = right eye; OS = left eye; OU = both eyes
PERRLA = pupils equal, round, and reactive to light and accommodation
EOMI= extra-ocular motion intact.

Eyes:

VA (visual acuity): OD, OS, & OU: 20/20 with pocket card. Fields intact to confrontation.  Conjunctiva pink; sclera anicteric.  PERRLA. EOMI. Bilateral red reflex.  Fundi benign with sharp discs & cup:disc ratio < 1:3.

 

THE NORMAL THOROUGH EYE EXAMINATION WRITE-UP

Eyes:

VA with Snellen card & corrective lenses: OD = 20/20; OS = 20/20; OU = 20/20.
Visual fields: all four quadrants intact & symmetrical to confrontation.
Unremarkable eyebrows, lids, and lashes.
Conjunctiva without pallor or lesions.
Sclera without icterus and benign.
Corneas and anterior chambers clear and without defects.
PERRLA
EOMI
Funduscopy: (+) full red reflex bilaterally. Discs sharp.  Cup:disc ratio < ½. Retinal vessels without narrowing, AV nicking, or hemorrhages.  No retinal infarcts, hemorrhages, or exudates.  Maculas benign.
