Instructions:
- Please record every patient encounter on the appropriate
page in your pocket syllabus. The degree of involvement
to meet the definition of "patient encounter"
is face-to-face with transfer of information.
Examples that meet the criteria: |
| 1. You work-up a patient in the ER, clinic or in the
hospital. |
| 2. You are present when another practitioner (MD, DO,
PA, ARNP) interviews and examines the patient, and an
assessment and plan are discussed. Rounding on new patients
in the hospital meets this criterion. Shadowing your preceptor
on new patients in clinic is sufficient. |

| Examples that don't meet the criteria: |
| 1. Discussion of a patient who is not physically present.
This could be in clinic or at a conference. |
| 2. Follow-up visits in clinic or on the wards in which
only a small part of the problem is discussed such as
response to an antiepileptic or headache medication. |
- You may record two problems for one patient if both are
significantly discussed.
Example: Patient has
both epilepsy and migraine headache. |
- Record your patient encounters at the end of weeks 1-3.
REQUIRED.
- We will monitor the log. Based on your responses, you
may receive an email at the beginning of the 4th week reminding
you to complete one or more required cases in order to meet
this objective. The pages are from Clinical Neurology, Lange,
6th edition.
