STUDENT REQUIREMENTS & INFORMATION

Prior to arriving for your clerkship: The confidentiality agreement and application below must be completed and received by Group Health TWO WEEKS prior to beginning your clerkship at any of the Group Health sites. You may mail or fax this paperwork to: Jung G. Kim, GME Coordinator at 206-326-3543 or Group Health Cooperative-FMR, 125 16th Ave. E., Mailstop CSB-160, Seattle, WA 98112. Clerkship orientation: Orientation is at the UW at 8:00AM on the first day of your clerkship. The room number is on the Didactics page. Room information may change at the end of each rotation, so be sure to check the table the weekend before you begin.

After orientation: Report to the preceptor at your assigned site. Driving directions are available on the Group Health website at www.ghc.org/locations/index.jhtml.

Equipment to bring: White coat and name tag, reflex hammer, tuning fork (128Hz), stethoscope, (Ophthalmoscope recommended).
