PRE-ARRIVAL REQUIREMENTS
Complete
the items below AT LEAST THREE WEEKS PRIOR to beginning your
clerkship.
- Send an email to Trudy Furberry
in the dean's office stating that you give permission to
forward your background check to Madigan. Trudy's email
is TRUDY@dommail.dom.washington.edu.
- Madigan needs your social security
number, date of birth, middle initial and country of citizenship.

to provide this information.
- Complete
the online New Employee Orientation module (only
complete NEO, not the BMAR module).

TRAVEL & HOUSING
Mileage reimbursement: You will receive
mileage reimbursement for one roundtrip to and from Madigan,
and for your trips back and forth to Seattle for weekly class
meetings. Contact GOWWAMI@dommail.dom.washington.edu
to arrange for reimbursement.
Housing: On base at Rainier Inn at Fort
Lewis Lodging. Check in any time the day before your clerkship
begins. Rainier Inn's phone number is 253-964-0211. They are
there 24 hours a day.
Driving directions: Interstate 5 goes right
through the post, Exit 120 will take you to the Fort Lewis
main gate, which is open 24 hours a day. You will need to
obtain a visitor's pass from the main gate before checking

FIRST DAY & ORIENTATION
Arrive at Madigan on the first day of your clerkship at 7:30AM
for in-processing. Check in with Kathy Rogers in the GME office,
8th floor of the nursing tower (phone: 253-968-1511). If you
are unable to meet with Ms. Rogers at this time, please contact
her to make other arrangements.
After processing with GME, go down to the Medical Mall, Neurology
Service and ask for Sharon Buckler (phone: 253-968-1399).
She will introduce you to the attendings and residents on
the service.
On Tuesday you will have a mandatory computer training session.
Ms. Rogers will provide you details for this session.
Equipment to bring: White coat and name
tag, reflex hammer, tuning fork (128Hz), stethoscope, (Ophthalmoscope
recommended).
Clerkship orientation: Because the clerkship
orientation conflicts with in-processing at Madigan, you will
need to do the clerkship orientation online. Go to the Didactics
page and view the Seattle orientation video. You willl need
to refer to your Pocket Syllabus (see below) when viewing
the video.
Pocket syllabus: A “pocket syllabus”,
referenced in the orientation video, will be mailed to you.

COURSE REQUIREMENTS
Didactics: Didactics take place on Thursdays,
1:00-4:00PM at the UW. The room schedule and details are on
the Didactics page. Room
information may change at the end of each rotation, so be
sure to check the table the weekend before you begin.
Cases: Two case write-ups are due each week.
The clerkship coordinator will email the assigned case numbers
to you on or before the first day of the clerkship. Further
information is available in the orientation video and on the
Cases page. Please
note that the first cases are due on the first Thursday of
your clerkship by 1:00PM Pacific fime.
Patient log: You are required to log in
the patients you have seen each week. The log is described
in the clerkship orientation video. Additional details are
on the Patient Log page.
Clinical & Bioethical CEX exercises: You
are required to do both a clinical and a bioethical mini-CEX
for this course. See the Mini-CEX
page for details.
Presentations: Presentation requirements
are described in the orientation video and are available on
the Presentations page.
Neuroanatomy: A neuroanatomy session will
usually be held at the UW. You will be notified of the time
and location when it is scheduled.
Final exam: This is described in the orientation
video and on the Final Exam page.
