PRE-ARRIVAL REQUIREMENTS

The items below must be completed AT LEAST TWO WEEKS PRIOR to beginning your clerkship. Fax the forms to Fawn Ross at 360-493-5524, or mail them to Ms. Ross at 525 Lilly Rd NE/PBP09, Olympia, WA 98506.

ORIENTATION, FIRST DAY & WHAT TO BRING

Clerkship orientation: Students in Olympia have the choice of attending the clerkship orientation at the UW or viewing the online orientation video. Please inform the clerkship coordinator of your choice at neural@u.washington.edu.

If you will attend the orientation in Seattle, see the Didactics page for the time and room number. (Room information may change at the end of each rotation, so be sure to check the table the weekend before you begin.) You will go to Dr. Bell's office after orientation. If you decide to view the orientation online, contact Dr. Bell's office to find out when you should arrive in the morning.

Equipment to bring: White coat and name tag, reflex hammer, tuning fork (128Hz), stethoscope, (Ophthalmoscope recommended).

TRAVEL & HOUSING

Travel reimbursement: You will receive mileage reimbursement for one roundtrip to and from Olympia, and for your trips back and forth to Seattle for weekly class meetings. Contact GOWWAMI@dommail.dom.washington.edu to arrange for reimbursement.

Driving directions to Olympia Neurology (click HERE for map):

Take I-5 South to Martin Way exit (#109) in Olympia.
Turn RIGHT at Martin Way
.
Travel on Martin Way (a little over a mile ) until Lilly Rd; turn RIGHT on Lilly Rd.
Olympia Neurology is in the St. Peter Professional Building at 525 Lilly Rd.
It is just past the entrance to St. Peter Hospital on the left side of the road.

Housing: Students will be housed in an extended stay hotel in Olympia with a kitchenette and workspace with Internet access. The clerkship coordinator will contact you with details. Some pet-friendly rooms are available, but availability will need to be confirmed. Please let the clerkship coordinator know as soon as possible if you would like to try to bring your pet.
