Diagnostic Report Checklist

Use this as a guide to review your reports BEFORE you turn them into the supervisor.

Submit this checklist via email when you turn in your report.

1.  Is the identifying information complete (name of client, date of birth, address, telephone number, etc.)?    
2.  Was the report ready on or before the due date?    
3. Is the chronological age correct?    
4.  Are the complaint and referral sources identified?    
5.  Does your report follow a fully developed history examination-summary/impressions-recommendations structure?    
6. Is only pertinent history information included?    
7. Is the report written in correct standard English?    
8. Is the information well organized and chronologically appropriate?    
9.  Do the impressions rest on the history and examination?    
10. Has the report been proofread?    
11. Are all the test results accurate?    
12. Are all tests fully interpreted?    
13. Does the summary/impressions section contain only the essential information (nature and severity of speech-language hearing problem, etiology, prognosis)?    
14. Is the report in correct form?    
15. Is the report free from grammatical errors?    
16. Are the recipients of the report identified (cc's)?    
17. Is there consistency in use of present, past, and/or future tense?    
18. Is the report concise?    
19. Have unnecessary words, meanderings, and repetitions been eliminated?    
20. Are recommendations for speech therapy, reevaluation, and follow-up included?    
21. Are recommendations based directly on objective interpretation of data?    
22. Is there a brief statement of recommendations given to the client or parents for alleviation of the problem?    

Each of the above questions should be answered "Yes." If you have answered "No," check that particular aspect of the report BEFORE you turn it into your supervisor.
