Small Group Facilitators

Weekly Case Write-ups

Weekly case write-ups will be due at 1330H each Friday. Please deliver them to Dr. Horn's mailbox in the H375 office suite. Copies should be provided to all group members including your facilitator.

Case write-ups should follow the format used in Pharmacy 560:

• 1 page SOAP Chart Note
• plus 3 to 4 pages of justification (font size = 12 points, 1" margins).

Complete sentence are not required for the Chart Notes; succinct, to the point phrases are preferred. The Chart Note will be graded (Pass/Fail) according to its accuracy and its acceptability as a document you would leave in a real patient's medical record.

Participation in
small groups and PALs

Participation in the small group and PAL section that you are registered in is required in order to pass the course. Facilitators and PAL section faculty will note student participation on a weekly basis for each student. Therefore, it is important to inform your facilitators or PAL section faculty member if you are unable to attend a session before the session begins. If a session is missed, at the discretion of the facilitator or PAL section faculty member.

Course grades will be determined as follows:

As noted above, participation in the small group and PAL section that you are registered in is required in order to receive a passing grade for the course.
